Outbreaks on Cruise Ships: Norovirus Affects Thousands in First Five Months of 2025

In the first five months of 2025, at least 17 cruise ships in the U.S. reported outbreaks of illness, primarily caused by the highly contagious Norovirus. Thousands of passengers and crew members were affected, raising concerns about hygiene and food safety practices on board. Major cruise lines, including Holland America, Princess Cruises, Viking, and Cunard, have been impacted, prompting a closer look at sanitation protocols.

Millions of people board cruise ships each year for relaxation and enjoyment, but some ships may harbor concerning hygiene secrets. The U.S. Centers for Disease Control and Prevention (CDC) reported that between January and May 2025, at least 17 cruise ships experienced outbreaks of illness. These outbreaks affected thousands of passengers and crew members, with Norovirus being the primary culprit in 13 cases, causing 1,568 illnesses. Other incidents included an E. coli outbreak on the Silver Ray ship and a suspected case of ciguatera poisoning from reef fish toxins on the Sea Cloud Spirit.

What is Norovirus?

Norovirus, often referred to as the “cruise ship virus” due to its rapid spread in confined spaces, causes vomiting, diarrhea, and stomach cramps. The virus can survive on surfaces for days and spreads through contaminated surfaces, direct contact, or food prepared by infected crew members. Between January 4 and May 3, 2025, 658 out of 10,748 passengers and 73 out of 4,172 crew members were infected, with diarrhea being the most common symptom.

How is an Outbreak Defined?

The CDC defines an outbreak as when at least 3% of passengers or crew on a ship report gastrointestinal illness symptoms. The CDC’s Vessel Sanitation Program (VSP) monitors health conditions on ships docking at U.S. ports, focusing on water and food safety, hygiene practices, waste management, and pest control.

Reevaluating Hygiene Protocols

The recent outbreaks have led to renewed scrutiny of hygiene and food safety practices on cruise ships. Major cruise lines are under pressure to enhance sanitation measures to prevent future incidents and ensure passenger safety.
